If you are involved in a Christian Community Theater (CCT)/Christian Youth Theater (CYT) class/camp/production/program/meeting (the "Activity"), you are hereby advised that our organization does not carry Workman’s Compensation for participants or volunteers. You, on behalf of yourself, representatives, heirs, next of kin (collectively hereafter “You”), hereby acknowledge and agree that the novel coronavirus infections (“Covid”) have been confirmed throughout the United States including Texas. If You should suffer an injury or illness (including without limitation, Covid) while participating in our program(s), You will be personally responsible for your medical or injury related expenses and You agree to indemnify and hold harmless CCT/CYT and/or their assignees with respect to an injury or illness and You hereby release waive and discharge CCT/CYT from any claims relating thereto. I am voluntarily participating in the aforementioned Activity and I am participating in the Activity at my own risk. I am aware of the risks associated with participating in the Activity. I further acknowledge that the Activity may involve a test of a person's physical and mental limits and may carry with it potential risks caused by the facilities in which the Activity is conducted, by the condition of the participants, equipment or vehicular traffic and/or by the actions of others, including but not limited to, participants, volunteers, spectators, instructors, Activity officials and Activity monitors, and/or producers of the Activity. I hereby authorize and consent that CCT/CYT-San Antonio shall have the absolute right to copyright, publish, use, sell, or assign any and all photographs, portraits or pictures, television spots, movie films, videotapes and/or sound recordings, or any part thereof, that have been taken of myself during CCT/CYT activities, or in which myself may be included in whole or in part, and I waive rights to royalties or other compensation arising out of or related to the use of said visual and aural media.
